Khushvika Healthcare Private Limited ("we") runs Vaccine Hub Clinic. This page explains what we do with the information you share with us.

What we collect

When you send an enquiry or booking we receive what you enter: name, phone, email, company, city, pincode and the vaccine you are asking about. When we vaccinate you we record the details needed for your vaccination record: name, age, vaccine, batch, date, consent and any reaction observed.

How we use it

Only to reply to you, deliver the vaccination, issue your certificate, invoice, and meet legal record-keeping requirements. For corporate drives, your employer receives a summary of who was vaccinated with which vaccine. We do not sell your information.

Forms on this site

Our forms open WhatsApp or your email app with your message filled in. Nothing is sent until you press send there.
